You may either complete the web form below to charge your gift to your credit card, or you may print and mail the PDF gift form.

You may choose to pay tribute to someone you know through an “in honor of” gift. Many choose to remember birthdays, anniversaries, and holidays or just show their support for a loved one through these gifts. You may also pay your respect to someone who has passed away through an “in memory of” gift. Many choose to do this in lieu of flowers.

With both types of gifts, we will send an acknowledgment to you and a separate notification (without the amount of the gift) to a person(s) designated by you to make them aware that a contribution was made by you in honor/memory of someone or something. CPNJ will send one of the notification cards below:
